As a new patient you will be offered an appointment within 24 hours of your initial call. We offer appointments between the hours of 7:00 a.m. and 6:00 p.m. Monday, Wednesday and Friday and 6:00 am to 6:00 pm Tuesday and Thursday.

We accept Medicare and most major insurances and our Patient Care Representative will answer any questions you have regarding your insurance coverage.

On your first appointment, we will do a complete evaluation in order to help us understand what is causing your pain and/or limitations. With you, we will tailor a program to meet your goals. We know that any pain is too much pain. Our goal is to address and correct your pain.

If you would like to fill out your paperwork before your first appointment, our New Patient packet can be downloaded in PDF or Microsoft Word format. The first two pages are a profile for you to fill out. The third page is an authorization to release information to your insurance company and the fourth page is our financial policy. Please read and sign each form and bring all four with you to your first appointment!
